Is this a printed book or a subscription?

It is a full-color paperback purchased once. There is no subscription or renewal. The individual price is $60 with U.S. shipping included.

How long does delivery take?

Books are printed to order. Allow approximately 2–3 weeks for delivery. A shipping notification is sent when the order ships; tracking is included when available.

Can a residency program order copies?
